The proposed changes expend the ICD codes used for identifying conditions.
Some of the changes are transplant codes which were only listed as transplant and not the primary organ system.
I've updated urls which have become invalid in the six-plus years since they were reported.
My hope is to make this version 1.0.6
An eventual v1.1.0 will have a deprecation note and links to a new package with pccc v2 and pccc v3 implementations which are orders of magnitude faster and more stable.
